Job Description:

Join our team as a Licensed Practical Nurse (LPN) in the Medically Ready for Discharge Unit (RDU), where you’ll find that the journey to discharge can be as rewarding as the destination! In this role, you’ll work alongside a collaborative multidisciplinary team, providing vital support to patients who are medically ready to go home but facing discharge delays due to various challenges.

Your daily tasks will include performing focused assessments under the guidance of the RN, developing and implementing targeted care plans, and monitoring patient progress. You will also have the chance to lead and supervise tasks delegated to other LPNs and assistive personnel, fostering a nurturing and efficient care environment. Your ability to communicate effectively will ensure that each patient's journey to discharge is smooth, while also allowing you to contribute to quality improvement initiatives that impact patient outcomes positively.
