In this engaging role, you will provide essential home health services to patients in their own living spaces. You'll be performing comprehensive assessments, navigating the changing landscape of patient needs, and creating tailored care plans while working in a cozy, community-focused setting. Interact with a diverse patient population, primarily geriatric, supporting them in their recovery post-acute treatment and managing chronic comorbidities.
