Job Description:

This travel healthcare contract is perfect for experienced Case Managers with a passion for adult acute hospital discharge planning. You'll be working on a fast-paced orthopedic/neuro surgical unit, where managing multiple discharges and admissions daily is a key part of the role. Expect to handle complex cases requiring UR experience, and to wear casual business attire as you support patient transitions. The schedule features 8-hour daytime shifts from 8:00 am to 4:30 pm, Monday through Friday, with some weekends, holidays, and no overtime unless approved by a supervisor. Your expertise will directly impact patient outcomes in a hospital setting that values efficiency and compassionate care.
